CCCSIG Health Benefits Program Membership Requirements
Requirements for Interested Non-Members:
Once notified by CCCSIG’s Health Benefits Broker Representative that the CCCSIG Health Benefits Committee recommends membership consideration to the CCCSIG Executive Committee, the following is required:
JPA Membership Documentation Requirements
The following financial items should be sent to CCCSIG electronically (jmorach@cccsig.org) as soon as possible in order for the Executive Committee to consider approval of JPA membership at their next meeting:
- Two most recent audited financial reports
- Adopted budget for the current fiscal year
- Most recent approved Interim Financial Report
The following items can be processed concurrently during review of the above documents:
- CCCSIG JPA Agreement (for review and signature)
- CCCSIG Bylaws (for review)
- Resolution to join the CCCSIG JPA (formal School Board approval requirement)
- Director Designation Letter (from Superintendent)
Assuming JPA membership is approved by the Executive Committee and the District’s Governing School Board, the following documents can be processed concurrently with the above:
- Membership Resolution to join the CCCSIG Health Benefits Program(formal School Board approval requirement)
- Committee Member Designation Letter
The JPA and HBP membership is a three-year commitment contingent upon mutual acceptance into the CCCSIG Health Benefits Program by the District and CCCSIG’s Executive Committee. The term of the first year is based on the Health Benefits Program insurance renewal process and may have an extended first year (e.g. 15 or 18 months).
The Health Benefits Committee meets monthly (based on business needs), generally on the second Friday from 10:00 – noon at CCCSIG’s offices in Pleasant Hill.
